CONTRACT CASE MANAGER/GROUP FACILITATOR

Project LIFT Services is a prevention and social justice agency providing behavioral health services, targeting the needs of Black teens and fathers most at-risk for homelessness, economic instability, and incarceration who live within Cuyahoga County. Project LIFT provides a continuum of services including linkage and referral to community resources, psychoeducational groups to build parenting skills, and prevention workshops to the community.

Position Overview

The Case Manager is responsible for facilitating support and therapeutic groups and maintaining a caseload of clients who would benefit from individual psychoeducation and resource linkage. The case manager will assess client’s progress, needs to ensure appropriate community referrals occurs in a timely manner.

Essential Duties

Participates in regular case coordination meetings to review client needs and plan for ongoing services or community linkage.

Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan in order to help the clients to achieve the stated goals and objectives.

Submits all required documentation in a timely manner to ensure program and billing compliance for all participants.

Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.

Performs other duties as assigned by the supervisor which are consistent with the position and in compliance with agency policies and procedures. Local travel may be necessary to meet the needs of clients.

Qualifications

Must possess a combination of mental health work experience or relevant education, and training which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties. Must have experience working with returning citizens, fathers, and court-involved teens. Bachelor degree in relevant human services field and LSW/LPC is preferred. This is a contract position, averaging 9 hours a week or less. Services may be provided during the day, evening, or weekend, depending on assignment.​
